Summer Day Camp, Fun in the Sun and Camp Santa Maria Registration

Please click here to register online. After you have registered for Summer Camps or Fun in the Sun, please fill out required forms and print and mail or fax to: YMCA Summer Camp Registrations, 5541 Central Avenue, Suite 135B, Boulder CO 80301 Fax: 303.544.7444 .

Summer Camp and Fun in the Sun supplemental forms
(Must be completed, signed and delivered to YMCA two weeks before attendance at program)
a. Health Emergency Form: Click here.
b. Summer Acknowledgement form: Click here.
    (Please download a copy of the Day Camp parent packet or the Fun in the Sun      parent packet to read before signing the Summer Acknowledgement form.)
c. Immunization Certification form
    (must be filled out and signed by your doctor): Click here.
d. Fun in the Sun Soccer form (deadline May 5) and Swim Lesson registration     forms (this form will be available soon).

After you have registered for Camp Santa Maria you will receive the Santa Maria Parent Guide and Santa Maria Camper forms in the mail. If you do not receive this information please contact camp at santamaria@ymcabv.org. Please fill out “camper forms” and mail to P.O. Box 1369 Bailey, CO 80421 by June 10th.
